The Senior Care Navigators Group is a full services Home Care Agency that provides options in care for their clients. It is owned and operated by nurses with over 40 years of experiences. As our healthcare system changes managing each individual care changes as well. Our Care Navigators know the value of personalized care planning for seniors, adults with disabilities and veterans as well as their family members and care providers.

Care Coordination
Improves the patient’s care experience by having efficient and effective communication and collaboration between providers and services. We make it easier for clients to navigate the healthcare system.
Support Employment
It is a model of employment created for people with severe disabilities, where these patient need special support to get success in a competitive work environment.
Transition & Support Living
We assist families and their loved one transition through all stages of health. Whether it be from a hospital, a rehabilitation center, or a long term care facility. We support our clients through each transition.
Medication Management
We oversee and coordinate each clients use of medications to ensure they are taken safely and effectively. Reviewing, monitoring and educating!
Alzheimer & Dementia Care Support
The responsibility of caring for a person with Alzheimer’s or Dementia can be a long, stressful, and intensely emotional journey. We strive to provide medical, emotional, and practical support and assistance to our clients and their family.
Personal Care Service
Our Professional team also provides assistance for those who need help with activities of daily living due to age, illness, or disability. Bathing, grooming, dressing, toileting, meal prep, or light housekeeping.
Respite Care
A temporary relief service for primary caregivers. Respite care allows caregivers a break from their caregiving responsibilities to rest, recharge, or attend to other personal needs, knowing their loved one is in safe and capable hands.
Chronic Care Management
We help reduce hospitalizations, improve health outcomes, and enhances the overall well-being of individuals with chronic conditions.
Transportation Services
This service is to help our clients who may have mobility limitations, lack access to private transportation, or unable to use public transportation for appointments, errands, etc.

ABOUT ME
Winsome McLeod, RN-CDP Coordinator
Winsome McLeod, RN-CPD is a caring and passionate healthcare provider with 28 years of experience as a nurse and corporate executive. Winsome learned the importance of respect, integrity, compassion, excellence and the value of human dignity during her family upbringing.
Winsome has served as an Executive Director/Administrator in the Senior Living industry since 2003, where she coordinated, management including, planning, staff training, counseling and budgeting for the senior Living Communities she served.
She is a certified Dementia Practitioner/Care Manager and specialized in Alzheimer's and Dementia Care since 2012. She also brings 10 years of service as a mental health nurse and a counselor for persons with dual diagnosis and substance abuse.
Her academic accomplishments include an AS in Nursing, a BS in Healthcare Administration, a Certified Dementia Practitioner, and a CDP Honorary Doctorate of Ministry. Winsome is a big believer in Christ which daily inspires her to provide quality care to the elderly, disabled and veterans.
